KLCA-FM and The Nevada Center For Reproductive Medicine PRESENTS

1. How to Enter the Contest:
(a) The KLCA-FM “HAVE A BABY - 2013” will begin on 1/14/13 and end on 2/10/13 at 11:59 PM PST. Contest is sponsored by: The Nevada Center For Reproductive Medicine.
(b) To participate in the contest, viewers must enter online at www.alice965.com and complete online survey and submit entry via email: 150 words or less on why they should be selected. All emails will be submitted to Dr. Whitten from the Nevada Center For Reproductive Medicine for review.
(d) Viewers may enter as many times as they wish. Entry deemed made by the holder of the email account.
(e) Broadcasts are subject to preemptions and scheduling changes. If such changes occur, they will be posted on KLCA-FM website.
2. Eligibility Restrictions:
(a) The contest is open to Northern Nevada residents who are 18 years of age or older. Employees of this station, employees of the sponsor, other radio stations in the Reno/Tahoe market, and their affiliates, subsidiaries and advertising agencies and their immediate family members or persons living in the same household are ineligible to participate or win.
(b) Viewers are eligible to win a KLCA-FM Sweepstakes only once every sixty days. Only one winner per household is permitted in any Sweepstakes. Viewers are eligible to win a prize valued at $600.00 or more only once every six months.
(c) All entries become the sole property of KLCA-FM and will not be returned. By entering, entrants agree to abide by these rules, and warrant and represent that their entry is their original work.
(d) Contestants are required to provide truthful information and Sponsor will reject and delete any entry that it discovers to be false or fraudulent. Sponsor will disqualify any entry from individuals who do not meet the eligibility requirements, and Sponsor will also delete any entry received from persons under the age of 18.
(a) The prize package being awarded through the Alice 96.5 promotion is defined as: fertility services provided by the Nevada Center for Reproductive Medicine (NCRM) up to a total of $10,000 dollars calculated by the standard current fees charged for the services utilized.
These services include consultations, in-house diagnostic services such as semen analysis and hysterosalpingogram, ultrasounds, inseminations, and in-house endocrine laboratory. Other services that may also be included involve assisted reproductive technologies such as ovum retrieval, oocyte identification, intracytoplasmic sperm injection, embryo culture, and embryo transfer.
Services performed outside of the NCRM office are not covered in the prize package. Some examples of items not covered include but are not limited to medications, anesthesia, and genetic testing.
Donor sperm samples and stipends to ovum donors are not included in the prize package.
There is no guarantee that a pregnancy will result from fertility treatment.
There is no guarantee that a healthy live born will result from fertility treatment. There are certain known risks to becoming pregnant and with certain fertility treatments. All applicable NCRM consent forms will be reviewed and must be signed before any treatment is performed.
There is no cash value to this prize package.
If services are not performed within one year from date awarded, the prize package coverage will expire and all applicable fees will apply.
Recipients of prize package hold harmless the physicians, owners, partners, managers, officers and employees of NCRM, Alice 96.5/KLCA, Americom and Reno Media Group and their affiliated entities from any obligation or responsibility to the offspring and to you that may occur from services rendered through this prize package. Recipients are electing to utilize services under their own will and decision.
(b) All other costs and expenses in connection with Prize Winner’s receipt and use of the prize are the winner’s sole responsibility.
(d) Contest prizes are not transferable. Prizes may not be substituted for or redeemed for cash. Station reserves the right to substitute prizes of comparable value.
4. Selection of Winners:
(a) Decisions of the contest sponsors with respect to the contest are final
(b) The Grand Prize Winner will be selected BY Dr. Whitten and The Nevada Center Fro Reproductive Medicine from all eligible entries received, on Feb. 9 – Feb. 12.
The Winner will be announced and contacted on the Bill and Connie Show on Alice 96.5 FM by the talents and Dr. Whitten on-air. If KLCA-FM is unable to contact a winner based on the information provided on the entry form, an alternate winner will be selected from all other eligible entries.
(a) Payment of all federal, state and local taxes are the sole responsibility of the winner and winners may receive an IRS Form 1099 or equivalent from Sponsor.
(b) By participating in the contest, the winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest, and to sign a publicity release, affidavit of eligibility and release of liability prior to acceptance of the prize. By accepting the prize, each winner grants to Sponsor the right to use the winner’s name, voice, picture and/or likeness for purposes of advertising and publicity in any and all media now known or hereafter invented, without further permission or additional compensation (except where prohibited by law). All expenses on receipt and use of prize, and all federal, state and local taxes, are the sole responsibility of winner. Winners, by acceptance of their prizes, agree to release KLCA-FM, Americom Broadcasting, Scott Comm, Reno Media Group, The Nevada Center Fro Reproductive Medicine, their respective parents and subsidiaries, officers, directors, shareholders and their respective advertising, promotion and production agencies from any and all liability, claims or actions of any kind whatsoever for injuries, damages or losses to persons and property which may be sustained in connection with the receipt, ownership or use of the prize or while preparing for, participating in, and/or traveling to any prize-related activity. Void where prohibited by law.
6. Sponsor reserves the right to make changes in the rules of the contest, which will become effective upon announcement. In the event of a misunderstanding, dispute, or human error, Americom Broadcasting, Scott Comm and Reno Media Group, The Nevada Center For Reproductive Medicine’s management will make the final determination of the prizewinner. By entering this contest, contestants agree to be bound by these rules and any Americom Broadcasting management decision concerning the contest.
7. Sponsor is not responsible for any typographical or other errors in the printing, the offering or the administration of the contest or in the announcement of the prize.
8. Failure to comply with the contest rules may result in a contestant’s disqualification.
